The card is a pale, translucent green covered by brass-colored printing on the obverse side and silver-colored solder lines on the reverse side. The obverse is covered with burnt-orange, baby-blue, black, red, and silver components; the three identical silver-colored, circular components are marked "SDS 306W-6913." The obverse substrate is marked "128095 D" on the left side, " SER SDS-65" on the right side, and "ASSY NO. 116029, BAND GATE, and BT11" along the bottom. "918" is also stamped at the bottom in black and is partially obscured by a round, pink sticker. The reverse substrate is marked with two black stamps, one reads "132" and the other reads "918 F."
