This is a W051 module with a dark green board and a tan plastic extraction handle attached by two rivets. "FLIP CHIP" is visible between the rivets. The component side is marked with three inspection stamps. The second stamp from the left reads, "Tested 18 A.M.T.1". "W" is printed in red throughout the module. "INDICATOR DRIVER W051D" is printed on the solder side.
