The card is a pale, translucent green covered by brass-colored printing on the obverse-side and silver-colored solder lines on the reverse-side. The obverse is covered with silver, black, and multi-colored components; two of the four silver-colored, circular components are marked on the top, the other two are marked on the side. The obverse substrate is marked "123194 D" on the left side, " SER SDS-65" on the right side, and "ASSY NO. 123193, RAMP GENERATOR, and OT10" along the bottom. A small black inspection stamp is also at the bottom. The reverse side is marked with four black stamps, one reads "30" and the another reads "914B."
