One (1) R151 module with green board and a red plastic extraction handle attached by two rivets. The handle reads "DIGITAL EQUIPMENT CORPORATION MAYNARD, MASSACHUSETTS" in raised letters on the solder-side. The board on the solder-side reads "BINARY TO OCTAL DECODER R 151 B" along the left-edge. "FLIP CHIP" can be read through the board between the rivets on the component-side. There are three black inspection stamps on the component-side; two circular and one square. A repeating "NP" pattern is printed on the board in red.
