The board is pale, translucent green covered by brass-colored printing on the obverse side and silver colored solder lines on the reverse side. The obverse is covered with burnt orange and silver components; a small, round paper sticker is attached to the bottoml eft corner. The obverse substrate is marked "124859F" on the left side, "CABLE DRIVER SER SDS-65" on the right side, and "ASSY NO. 124629 and AT12" along the bottom. The reverse side is marked with three black inspection stamps. The bottom of the card is damaged where two factory made 1/8" holes have broken through to the edge. The broken pieces not extant.
